ELIGIBILTY REQUIREMENTS


New hire and rehire associates are eligible to participate in the ImmunityBio Health & Welfare benefit program as defined below, effective the 1st of the month after date of hire.

  • Regular full-time associates (those scheduled to work at least 30 hours per week) may elect from the full offering of benefits, including medical, dental, vision, FSA, and voluntary life/AD&D coverage for yourself and your eligible dependents:
    • Legal spouse or domestic partner with whom you have lawfully registered into a domestic partnership in a state or municipality that provides for such registration.
    • Natural child, stepchild, or adopted child of the associate or spouse/eligible domestic partner, up to age 26. (Includes child for whom the associate or eligible spouse/domestic partner is the legal guardian).
  • Part-time associates (those scheduled to work less than 30 hours per week) are not eligible to participate in this program, unless specifically provided by the Affordable Care Act.
  • Full-time IB Temporary associates who work at least 30 hours per week may elect medical coverage for yourself and your dependent children as defined above in accordance with the Affordable Care Act.
    • If you reside in CA, you are eligible to participate in the BlueShield HMO plan
    • If you reside outside of CA, you are eligible to participate in the BlueShield PPO plan

Qualifying Life Event


Experiencing a Qualifying Life Event during the plan year?

If you experience a Qualifying Life Event (QLE) during the calendar year, you can change your benefits. Qualified family status changes allow you to add or drop dependents or change your level of coverage. Most common QLEs are:

  • Marriage, divorce, or legal separation
  • Gain or loss of an eligible dependent for reasons such as birth, adoption, court order, disability, death, marriage, or dependent child turning 26
  • Changes in your spouse’s employment resulting in loss or gain of coverage
  • Changes in your spouse’s benefit coverage with another employer that affects benefit eligibility
  • Moving in or out of a different state

Within 30 days of the QLE:

  • You must make any dependent, plan and/or coverage changes to your existing elections via the Benefitsolver Personal Benefits Portal
  • You will be required to provide proof of the qualifying event to the Benefits Team at benefits@immunitybio.com.
  • The change to your benefit election(s) must be consistent with your QLE change.
