New hire and rehire associates are eligible to participate in the ImmunityBio Health & Welfare benefit program as defined below, effective the 1^st of the month after date of hire.

• Regular full-time associates (those scheduled to work at least 30 hours per week) may elect from the full offering of benefits, including medical, dental, vision, FSA, and voluntary life/AD&D coverage for yourself and your eligible dependents:
  • Legal spouse or domestic partner with whom you have lawfully registered into a domestic partnership in a state or municipality that provides for such registration.
  • Natural child, stepchild, or adopted child of the associate or spouse/eligible domestic partner, up to age 26. (Includes a child for whom the associate or eligible spouse/domestic partner is the legal guardian).
• Part-time associates (those scheduled to work less than 30 hours per week) are not eligible to participate in this program, unless specifically provided by the Affordable Care Act.
• Full-time IB Temporary associates who work at least 30 hours per week may elect medical coverage for yourself and your dependent children as defined above in accordance with the Affordable Care Act.
  • If you reside in CA, you are eligible to participate in the BlueShield HMO plan.
  • If you reside outside of CA, you are eligible to participate in the BlueShield PPO plan.

An associate’s eligible Dependents may also participate in the Plan. An eligible Dependent is considered to be:

• Legal spouse or domestic partner with whom the associate has lawfully registered into a domestic partnership in a state or municipality that provides for such registration.
• Natural child, stepchild, or adopted child of the associate or legal spouse/eligible domestic partner, up to age 26. (Includes child for whom the associate or eligible spouse/domestic partner is the legal guardian).
• An unmarried child aged 26 or over who is or becomes disabled and is the legal dependent of the associate or legal spouse/eligible domestic partner.

To be eligible for coverage under the Plan:

• A Dependent must reside within the United States
• The associate must be enrolled in the Plan in order to enroll Dependents
• If the associate and legal spouse/registered domestic partner are both covered under the Company’s Benefit Plan
  • Each associate can be enrolled as a Participant; or
  • One associate can enroll as a Participant with the other associate covered as a Dependent
  • An associate cannot be listed as both a Participant and a Dependent in the Plan
