- Basic Life/AD&D | Two times earnings up to a maximum of $1,000,000
- Voluntary Life/AD&D | One, two, or three times annual earnings not to exceed the lesser of three times annual earnings or $1,000,000
- Evidence of Insurability | Required if you apply after your initial enrollment. You are guaranteed coverage up to the lesser of 3 times earnings or $200,000
